Social Worker, Care Management

Location: Honolulu, HI

Job Description

The HPH Medical Group is comprised of approximately 600 employed physicians and allied health professionals. Together with our four hospitals (Kapi‘olani, Pali Momi, Straub, and Wilcox) and more than 50 convenient clinic locations statewide, our nonprofit health system is one of the state’s largest health care providers. Our network of doctors and specialists work together to provide a distinctive and effective model of coordinated care for maintaining the health and wellness of our patients.

Internal Medicine physicians specialize in the diagnosis, management and nonsurgical treatment of unusual or serious diseases. Our specialists in internal medicine or “Internists,” have a lengthy clinical and scientific training in their areas of medical interest and special expertise in management of patients with complex, interacting medical conditions. Services include immunization, preventive care and patient education through a long-term personal relationship with their patients. Our caring and professional team also coordinates the efforts of other medical and surgical specialists as needed to assure our patients’ best possible health.

If you are compassionate and empathetic, you could be our next Social Worker and provide invaluable counseling and guidance services to patients and their families undergoing traumatic or challenging situations. Your responsibilities include providing assessment, counseling, clinical project management support, clinical work groups support, and information and referral to patients and their families. We are looking for someone sensitive and perceptive, with a strong sense of community care and social service, who shares our commitment to creating a healthier Hawai‘i.
