RN Case Manager for Case Management Hawai’i

Candidate must live on Oahu, Kauai, Maui, or Hawaii.

Case Management Hawaii (CMHI) was founded by health care professionals in order to improve the quality of life for the patients we serve through increased communication, access to options, education, oversight and follow-up.  We strive to improve health outcomes, medication adherence, and appropriate use of healthcare resources.

RN Case Manager:  Ideal candidates should be registered nurses that are licensed to practice in the state of Hawaii, and have a passion for direct patient care in the community setting.  Experience in providing direct patient care is desirable.  The candidate must have a strong independent work ethic and self-management skills.  The candidate must be reliable and a self-starter, extremely motivated, detail oriented, and understand basic computer skills. The candidate must have great communication skills and the ability to work at a fast pace, and work under pressure at times.  The candidate must have a reliable mode of transportation, mobile device and computer.

The Job: This position requires meeting with various patients face-to-face in care homes throughout Oahu.  This position also requires providing education, guidance, oversight and care coordination to the patients you serve.  This position will allow you to have a flexible schedule while working from home, however you will need to conduct face-to-face visits for direct patient care.

Assess, plan, implement, and coordinate care

Abide by all policies and procedures to assure the safety and security of the patients we serve

Collaborate with physicians, care providers & interdisciplinary teams

Focus on preventive care

Provide client-centered, family focused, and culturally competent services

Practice HIPAA compliant rules

Professional attire, appearance and conduct at all time

Utilize provided tools & training to maximize potential

In order to join our success, you will need:

A reliable mode of transportation, mobile device and computer

Basic computer skills

Desire to work independently in the community setting

RN license with the ability to work in the state of Hawaii

Strong independent work ethic & self-management skills

Desired Qualifications

Experience in acute care or sub-acute care

Case management

Hospice care

Wound care
