Case Manager - Community Based Outreach & Advocacy

Location: Kalihi, Oahu

Hale Kipa is looking for an individual to provide case management services to youth and families, which include identifying the needs of the youth and family and developing a service plan.  It is imperative that this individual be able to develop relationships with youth and find ways to engage them in service participation. Developing a relationship with the family is also key to this position, as well as being able to work in collaboration with any other identified individuals who play a part in providing services to the youth.  Transportation is also an essential function of the job as youth often need assistance getting to activities and various appointments.  There may be an occasional need to do outreach to schools and community groups to identify the youth.

The Case Manager also needs to have some knowledge of technology since all of the youth/family information is entered and stored on an electronic health record database.  Being able to communicate effectively both verbally and in writing are important skill, in addition to being timely and meeting deadlines for documentation submittal.  

  • Payrate: $40,000 annually
  • Qualifications:  Bachelor's degree in a related field such as Social Work, Psychology, Criminal Justice, or Sociology is required.  At least one year of experience with youth is also required.
  • Requirements:  Vehicle, valid driver's license, auto insurance, and safety check.
