Distinguished Alumni Award nominations can be submitted electronically using this online form, or print nominations can be mailed in or delivered in person. All mailed nominations and materials must be postmarked by the new extended deadline of Monday, Nov. 10, 2014. Deliveries and online submissions must also be received by the new extended deadline of 5 p.m. (HST) on Monday, Nov. 10, 2014. Submissions received after these deadlines will not be considered. To be considered, nominations should include:
- Completed nomination form
- One 8.5" x 11" page for each of the three nominee qualifications to be evaluated: Service to the University of Hawai'i, Service to the Community and Professional Accomplishments
You may also submit up to four additional 8.5" x 11" pages of supporting information, thus totaling no more than seven pages. We will evaluate nominees using only information provided within that seven-page limit. We will not consider oversized pages or physical collateral, such as CDs, DVDs, video clips, books, magazine tear sheets, plaques, trophies, awards and proclamations.
Please submit this form once. Each submission will replace any previously submitted forms.
