Case Manager - Guide on the Side
Location: Honolulu
Hale Kipa is looking for a full-time Guide on the Side Case Manager to provide services to young adults who are homeless. Services include: identifying eligible young adults, securing housing, case management, independent living and social-skill building, assistance with employment/vocational/educational resources, and any other services needed to support the young adults to successfully live in the community. This position is part of a team approach.
Must have good verbal and written communication skills and be able to navigate our electronic health record. Timely documentation is essential.
- Payrate: $40,000 annually
- Qualifications: Bachelor’s Degree in a related field such as Social Work, Psychology, Sociology, Criminal Justice, etc.; and one year of experience with youth/young adults, is required.
- Requirements: vehicle, current driver’s license, auto insurance, and safety check.
